This is AMAZING! I've seen this floating around the internet a few times, but never knew who made it or when. I finally ran into it again and did a reverse image search to find the artist. I was so shocked to be brought to DA. See, you catch the period so well that by mistake I believed that this was an illustration or woodcut made about 100 years ago by someone who would have been around at the time of the plague. You mentioned in another comment that you could have been an illustrator 100 years ago, and I can defiantly see the influence. 

This is the kind of artwork I expect to see in a history book about the time period and disease. This is great. I love the cold, dulled, sad colors. Having looked at your uncolored version, I definitely think color is what this picture really needed for the emotional weight of the children being taken away in the night by Nanny Death really needed to really send the horror home. With the color differences, it was abundantly clear what the difference between the girl and her family truly was and why she was leaving with Death.

I also really like the imagery of death as a sort of Nanny to lead the children away. It's still horribly sad to see the cold girl and buggy leave with the reaper, but it's interesting to see the reaper as something more of a grim caretaker instead of a totally fearful monster.

Making 1918 the house number was a good call. It makes it abundantly obvious what is really being referenced here, and for anyone finding this who didn't immediately get the reference (like me) it was enough of a clue to get them (me) to go research the 1918 flu outbreak.

This is by far one of my favorite artworks ever, on DA or elsewhere else. Fantastic work.
